New Patient Forms
We require all new prospective patients to complete a short new patient application form. You may complete the form online and it will be saved and sent to our office or you may also print the completed form and bring it to our office. Once we receive the completed form, please allow 2-3 days for your application to be reviewed. Please select the "Apply to Become a Patient" button below, fill out the form completely, and it will be securely saved or you may print the completed form and return it to our office in person.

What is acute care?

Urgency Matters. When you are over 6o, waiting too long to seek medical attention when you're unwell may be akin to witnessing a train crash in slow motion. Seek care promptly. Don’t let the train derail.
Acute care involves medical services to diagnose and treat acute illnesses like colds and flu and minor injuries. These conditions typically require only short-term care to help you achieve a full recovery.

The Three Rivers AMA providers also offer acute care treatments for chronic conditions like asthma, which causes shortness of breath, or allergies that cause respiratory issues that require urgent medical attention.

The Three Rivers AMA providers also offer a variety of onsite testing services, including lab blood draws, to confirm your diagnosis, and customize a care plan to help you feel better and prevent the condition from worsening.

Acute care visits are available in the office and the providers can also evaluate your symptoms through an online telehealth platform if you’re unable to travel to the office.

As a patient at Three Rivers Area Medical Associates, you will also have access to special programs that are focused on keeping you healthy and providing support for your independent daily living.
